What to expect:

Your initial physical therapy appointment allows us to get to know one another. Your forms and any medical records will be reviewed and a thorough evaluation will be performed. We will discuss the findings and set goals together for your treatment. Treatment will be initiated, including a home exercise program. There will be time for questions and discussion during the 60 minute session.


Eby Physical Therapy is now open for both in-person and telehealth appointments.

FAQ's

+ Do You Take Insurance?

Eby Physical Therapy is an out-of-network Insurance provider with all insurance companies. If you have out-of-network benefits, we will provide all documentation that you will need to submit for reimbursement. Payment is expected at the time of treatment.

Eby Physical Therapy is not a Medicare provider, and Medicare does not allow out-of-network services. We are legally prevented from treating Medicare patients for “normally covered services.” If you are covered under Medicare and have questions or would like to discuss treatment options, please call us at 212-457-1267.

+ Do I Need a Doctor’s Referral?

New York State law allows you to be treated without a referral for 10 sessions or 30 days, whichever comes first. Please be aware that your insurance may not reimburse you for services provided without a referral.

+ What forms of payment do you accept?

Payment may be made by check, credit card, or cash.

+ What should I bring to my first appointment?

Please bring completed forms and any relevant medical information (x-ray reports, MRI, etc.). Wear or bring clothing that allows visualization of the area to be treated. For treatment of the foot/ankle and for orthotic appointments, the knee must be visible. For orthotic appointments, please bring any current/past orthotics as well as typical shoes that you wear during the day.
